All medical offices need to charge an initial one-time higher office charge for new patients. This covers the additional costs of creating a new office chart and setting up a billing account. If necessary, you may be asked to sign a Release of Medical Information form so that we can obtain your previous medical records from other physicians.
For patients who do not have health insurance and those for whom we do not bill their insurance, we offer a 10% discount if services are paid in full at the time of their visit. Ask our office staff to assist you.
As a courtesy to all our patients, we will bill many primary insurance companies for you. Please check with our office staff for information regarding which companies we work with. However, you are expected to pay your co-pay at the time of your visit.
We will bill Medicare for you. You will receive a statement after Medicare has processed your claim, noting that Medicare has paid its portion of the charges, applied them to your deductible or transferred them to the patient responsibility. If you have supplemental insurance, we will also bill your Medicare Supplement for you. You will receive a statement from our office after Medicare and your secondary insurance have processed your claim.
We see Medicaid patients for whom we are their primary provider. You are required to present a current Medicaid card to the receptionist upon arrival at each visit. All Medicaid co-pays are due at the time of service.
If you need a refill on a previously prescribed medication, please contact your pharmacy. The pharmacist will fax us your request along with the current dosages and medications for your healthcare provider's approval. If the time elapsed since your last prescription refill is greater than one year, you will be asked to make an appointment before your medications are refilled. Please remember to allow two working days from the time we receive the fax from the pharmacist until your refill request will be processed.
We attempt to see all patients, when possible, on the same day of your phone call. Same-day appointments are usually available for emergencies and urgent or sudden illness. We ask that you please call first so we can advise you on the best approach to ensure appropriate medical care. We understand that circumstances may arise causing you to cancel or reschedule your appointment. However, please be considerate of our patients that need to be seen sooner by notifying our office at least 24 hours prior to your scheduled appointment time.
It is the policy of our practice that all physicians and staff preserve the confidentiality to the highest degree possible of all health information pertaining to our patients. We adhere to the standards set forth in the Notice of Privacy Practices (HIPPA) in conformance with all state and federal laws.
